Walking Home Form - Teignmouth Primary

Dear Parents/Carers of year 3, 4, 5 and 6 children,

If your child is allowed to leave school at the end of the day without being collected, please complete the form below. This option is only available to children in Year 3, 4, 5 and 6.

If you have already completed a form previously, you do not need to submit it again. Children will not be allowed to walk home without this consent form being completed and will remain with the teacher until collected.

This information may also be used for other school events where separate letters with permission slips have not been signed, such as the disco.

Please note that the school office must be contacted if you wish to change the permission you’ve given.
